What is the dosing for fondaparinux?

Fondaparinux 5 mg subcutaneously once daily for patients with body weight < 50 kg; 7.5 mg subcutaneously once daily for patients with body weight 50-100 kg. Consider increasing dose to 10 mg subcutaneously once daily for patients with body weight > 100 kg.
