  • Rivaroxaban 15 mg orally twice daily for 3 weeks
  • Apixaban 10 mg orally twice daily for 1 week
  • Edoxaban 60 mg orally once daily following ≥ 5 days of parenteral anticoagulation lead-in
  • Dabigatran 150 mg orally twice daily following ≥ 5 days of parenteral anticoagulation lead-in
