Why evidence-based resources cannot replace clinical reasoning
Modern clinicians practice with unprecedented informational access.
With a few keystrokes, a physician can retrieve continuously updated summaries of the medical literature through platforms such as UpToDate, DynaMed, and newer AI-assisted tools such as OpenEvidence.
These resources are remarkable.
They compress thousands of studies into concise recommendations.
They update continuously.
They place an enormous body of knowledge within immediate reach.
Used well, they improve care.
But their usefulness depends on something that precedes them.
Thinking.
Evidence answers questions
Evidence platforms are designed to answer questions.
They are not designed to determine which question should be asked.
That task belongs to the clinician.
A search engine, no matter how sophisticated, requires an entry point.
Without a well-formed question, the search becomes directionless.
Clinical reasoning provides that structure.
The sequence of clinical reasoning
Clinical reasoning unfolds in phases.
Orientation.
Thinking.
Execution.
Evidence belongs primarily in the last.
Orientation
Orientation asks a deceptively simple question.
What kind of situation is this?
Not the diagnosis.
The terrain.
Acute or chronic.
Localized or systemic.
Stable or unstable.
Which physiological systems are involved.
Which time scales matter.
Which risks cannot be missed.
Orientation transforms scattered clinical data into a recognizable problem space.
Without this step, the clinician does not yet know what they are looking for.
And if the problem is undefined, searching the literature will not clarify it.
Thinking
Thinking constructs the internal model.
Hypotheses form.
Mechanisms are considered.
Probabilities are weighed.
The clinician begins to ask:
What explanations fit this pattern?
What information is missing?
What possibilities are dangerous enough that they must be excluded?
This phase converts a vague clinical problem into a structured question.
Not:
How do you treat thrombocytopenia?
But:
How should immune thrombocytopenia be managed in a patient whose bleeding risk and thrombotic risk compete with each other?
Only now is the question ready for evidence.
Execution
Execution is where evidence enters.
The clinician consults summaries, reviews recommendations, and examines the literature.
Evidence platforms excel here.
They synthesize enormous bodies of research that would otherwise take hours or days to assemble.
And the evidence they provide guides many forms of action:
diagnostic testing
risk stratification
treatment decisions
monitoring strategies
But even here, interpretation remains necessary.
Clinical trials study populations.
Patients arrive as individuals.
The clinician must still decide:
Does this patient resemble the studied population?
Are the risks symmetrical?
Are the tradeoffs acceptable?
Evidence informs execution.
It helps determine what to do next.
But it does not replace the reasoning that made the question meaningful in the first place.
A posture problem

The concern in that essay is cultural.
The concern here is cognitive.
Evidence becomes dangerous when it enters the reasoning process too early.
When clinicians search before orienting and thinking, their reasoning begins to inherit the structure of the resource rather than constructing its own.
Most evidence platforms are organized around diagnoses already assumed.
Searching them too early quietly assumes that the diagnostic frame is already correct.
The result is algorithmic medicine.
Sometimes correct.
Often misapplied.
What remains uniquely clinical
Execution guided by evidence can appear deceptively straightforward.
Look up the condition.
Follow the recommendation.
Order the test.
Start the therapy.
If clinical reasoning began and ended here, medicine would largely become a matter of implementation.
Any well-educated person with access to the same evidence platforms could, in principle, carry out those steps.
In fact, one could imagine a sufficiently sophisticated system doing the same.
A robot could retrieve the guideline.
Identify the recommended test.
Initiate the protocol.
Execution is the part of medicine most susceptible to automation.
But medicine does not begin with execution.
It begins with orientation.
What kind of situation is this?
And with thinking.
What explanations fit this pattern?
What risks matter most?
What question should be asked?
A robot can retrieve evidence.
It cannot decide what question should be asked in the first place.
Those acts determine which evidence is relevant and which recommendation applies.
Without orientation and thinking, evidence platforms would simply return answers to the wrong question.
The real educational challenge
Trainees today learn medicine in an environment saturated with information tools.
This is a gift.
But it changes what must be taught.
In previous generations, trainees needed help finding evidence.
Today they need help framing questions.
Teaching clinicians to think before searching may be one of the most important educational tasks in modern medicine.
Otherwise we risk confusing access to knowledge with understanding.
Evidence can guide action.
Thinking must guide evidence.
