What is the dose of IV argatroban in heparin-induced thrombocytopenia (HIT)?

  • Normal organ function → 2 μg/kg/min 
  • Liver dysfunction (bilirubin >1.5 mg/dL) → 0.5-1.2 μg/kg/min 
  • Heart failure, anasarca, postcardiac surgery → 0.5-1.2 μg/kg/min
