A 25-year-old Portuguese-speaking man with no past medical history presents with a one-month history of worsening intermittent abdominal pain in his right lower and left lower quadrants associated with nausea, night sweats, five-pound weight loss, and intermittent blurry vision in his right eye. Additionally he complains of epistaxis and easy bruising. The patient does not take any home medications, is married with 2 children, works in landscaping, denies tobacco, alcohol, or drug use, and has a family history remarkable for thyroid cancer in his mother.
